Affidavit for Name Discrepancies
Question: What should I do if my name is different in
my passport and educational documents?
Answer: If your name on your passport does not
exactly match the name on your educational documents, you will need to
obtain an affidavit. This affidavit serves as a legal declaration
confirming that both names belong to the same person.
How to Obtain an Affidavit:
Contact
a Lawyer or Notary: You will need to approach a local lawyer or a
notary public to prepare and legalize the affidavit.
Bond
Paper: Ensure the affidavit is printed on a bond paper with a
stamp duty value of ₹10 to ₹100 rupees.
Notarization:
The affidavit must be notarized by the lawyer or notary public.
This legalizes the document.
What the Affidavit Should Include:
While I cannot provide a direct sample, a typical affidavit
for a name discrepancy should include:
Your
Full Name: As it appears on your passport.
Your
Other Full Name: As it appears on your educational documents.
A
Clear Statement: Declaring that both names refer to one and the same
person.
Reason
for Discrepancy (Optional but helpful): Briefly explain why the names
are different (e.g., maiden name vs. married name, spelling error, etc.).
Supporting
Details: Mention the specific educational documents where the name
differs.
Date
and Place: The date and location where the affidavit is being
executed.
Your
Signature: Signed in the presence of the lawyer/notary.
Lawyer/Notary
Seal and Signature: The official seal and signature of the lawyer or
notary public.
Note: Always consult with the specific authority or
organization requesting the affidavit to confirm any particular requirements
they might have for its content or format.
